NATURE OF WORK:

Responsible for coordinating care for customer who meet the requirements for the Alaska Medical Assistance Waiver Program.

KNOWLEDGE, SKILLS, AND ABILITIES:
Customer Care & Relationships: Creates, develops, and nurtures culturally appropriate interactions and connections with each other, customer-owners, and the community. Completes the intake and screening process. Conducts an informal evaluation to determine the waiver applicant’s financial eligibility and whether a full assessment is warranted. Refers the applicant to the Alaska Division of Public Assistance for Medical Assistance if there are not a current Medical Assistance recipient. Forwards screening information to the appropriate waiver administrative unit as part of the waiver approval process. Completes and documents a comprehensive assessment. Conducts a written comprehensive assessment of the applicant’s physical, emotional, social and cognitive functioning to determine the need for services and level of care required. Arranges for the following qualified persons to participate in the assessment, if necessary: Healthcare professional, social service professional, family caregiver or designated representative, appropriate hospital or nursing facility personnel, any other provider relevant to the applicant’s needs. Utilizes the standard assessment to document the applicants needs. Documents their recommendations and ensures the documentation of other qualified persons for the level of care for the applicant. Develops a plan of care for the applicant based on the needs identified in the assessment. Specifies in the plan of care: the applicant’s location; all services to be provided; frequency, amount, project duration and cost of each service; who will provide the services, and the expected outcome goals. Forwards the plan of care with the screening and assessment to the waiver administrative unit for review and approval. Informs the providers of services when approval of the level of care, plan of care, and eligibility for waiver participation (waiver slot) is received. Reviews and updates the assessment and plan of care annually or when a change in the recipient’s condition warrants it. Coordinates formal and informal supports. Gains access to other plan services and entitlements. Provides assistance with housing. Monitors services to ensure quality or care and utilization. Conducts case reviews to assess participant’s progress and satisfaction in meeting goals and objectives established in the plan of care. Recommends modifications, transfers or terminations when appropriate. Provides for crisis intervention, client advocacy or assistance, consultation, networking, and family support. Contacts the recipient twice a month. At least one contact must be face to face unless this requirement is waived by the appropriate managing agency. Knowledge of scope of customer-owner care services provided by other clinical service lines and the referral process. Communications & Teamwork: Shares and receives information, opinions, concerns, and feedback in a supportive manner. Works collaboratively by building bridges, and creating rapport with team members within departments and across the organization. Ability to participate in a multidisciplinary clinical team. Improvement & Innovation: Makes meaningful improvement to services, program, and processes and/or organizational effectiveness that creates new value for customer-owners and employees. Knowledge of clinical quality improvement training. Knowledge of effective teaching processes and principles in planning, providing and evaluating optimum health care. Knowledge of quality management and improvement processes to comply with internal and external standards, i.e., JCAHO. Workforce Development, Skills, and Abilities: Seeks out additional learning opportunities to continue to develop the technical and professional skills needed now and in the future. Takes responsibility for all work activities and personal actions by following through on commitments. Knowledge of home health care services. Knowledge of general medical procedures and the ability to anticipate complications and the indicated therapeutic interventions. Knowledge of clinic practices and ability to act as a resource for clinic personnel and assist the providers as necessary. Proficiency in the use of appropriate computer based software and office equipment. Knowledge of 1974 Privacy Act is required as use of customer-owner record is an integral part of the position and privacy of individuals must be protected to the fullest. Ability to maintain quality safety and infection control standards.

QUALIFICATIONS:
SCF programs are established for the purpose of serving a primary population comprised of Alaska Natives who are affiliated with the Cook Inlet Region Inc. (CIRI), and Alaska Natives and American Indians within SCF’s geographical service area. Employees should have a thorough understanding of the cultures and the needs of the CIRI members, and of the general culture and needs of Alaska Natives and American Indians. Such knowledge is critical in order to ensure the achievement of the mission and vision of SCF: working together with the Native community to provide the best services with Native staff who value the family and family wellness as the heart of the Native community. Meets the qualifications outlined in the job descriptions for Social Worker-BSW, Licensed Practical Nurse and/or certification as a care coordinator for the Choice Medicaid Waiver Program through the Division of Senior and Disability Services of the State of Alaska. Two (2) years professional experience required, if applicable. Current license in the State of Alaska. Basic Life Support (BLS) certification is required or must acquire within thirty (30) days of hire.
